Refund policy

Cancellations must be made at least 3 hours before the session for it to be rescheduled. Later than that, the session is lost.

Liability Waiver, Medical Release, and Code of ConductRead

Assumption of Risk

I understand that soccer is a physical activity that involves risk of injury, including but not limited to sprains, fractures, concussions, and other injuries. I voluntarily allow my child to participate and accept these risks on behalf of myself and my child.

Release of Liability

In consideration of my child's participation, I release, waive, and agree not to sue 4-session field package, its organizers, coaches, staff, volunteers, and the owners of any facility where the league is played from any claims arising from my child's participation, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an injury or medical emergency, I authorize league staff to seek emergency medical treatment for my child if I cannot be reached. I agree to be responsible for any resulting medical costs.

Concussion Acknowledgment

I understand that any player showing signs of a concussion will be removed from play and may not return until cleared by a licensed healthcare provider.

Health Statement

I confirm that my child is physically able to participate, and I have disclosed any medical conditions the league should know about.

Photo and Video Release

If I have checked the photo permission box, I grant 4-session field package permission to use photos and videos of my child for league promotion and social media.

Parent Code of Conduct

I agree to encourage all players, respect coaches and referees, keep sideline comments positive, and pick up my child on time. The league may remove anyone from events for unsafe or abusive behavior, with no refund.
